The main cost drivers
Licensing model: some vendors charge per bed, some per concurrent user, some per module, and many now offer subscriptions. Scope: a clinic needing front desk, billing and pharmacy costs less than a multi-specialty hospital needing every module.
Implementation: configuration, data migration, training and hypercare are real work. Some vendors price them separately, others bundle them. Hosting and support: cloud subscriptions typically include hosting; on-premise needs servers, backups and IT staff.
Hidden costs to ask about
Ask whether these are included: data migration from your old system, custom reports, integrations (ABDM, NHCX, lab machines, PACS), additional users or branches, upgrades, and after-hours support. Get everything in writing.
How to compare vendors
